New Patient Form

New patient procedure/policy:

Thank you for selecting us to consider your dental needs. We are pleased you have chosen our office and look forward to meeting you.

We have designed this website to assist you with some questions you may have about our office.

Your first visit will begin with a brief consultation with Dr. Kimberly Fee, after which a member of our team will take any necessary records, which may include radiographs and study models. The doctor will then complete a thorough clinical exam, including an oral cancer screening, head and neck exam, and periodontal probing.

At this point we may encourage you to set aside a treatment planning appointment where you and the doctor can examine your dental records together and plan for any necessary treatment.

Insurance/payment policy:

We accept cash, checks, and all credit cards. Payment is expected at the time of each visit. In special circumstances, we may be able to make other arrangements. We are happy to assist you with the paperwork necessary to bill your insurance. While it is not our policy to accept assignment from your insurance company (with the exception of Delta Dental), we do have some flexibility with large scale treatment plans. Please discuss your financial concerns or arrangements with our financial coordinator. (Please find our Written Financial Policy in the FORMS section.)
Financial Policy

Cancellation policy:

We have made a commitment to provide you with conscientious, quality care, and to do so as comfortably as possible. It is important to ask for a commitment from you as well. We carefully plan your treatment with you and specifically for you. Incomplete treatment can lead to complications, misunderstandings, additional disease, and loss of teeth. We believe that once the commitment to treatment has been made, it should be kept. Your appointment is time reserved especially for you. We will make every effort to be on time and ask the same courtesy of you. To have an opportunity to fill an available appointment, we ask that you give us at least 24 hours notice cancellation. Please be aware that we may charge for broken appointments or cancellations with less than 24 hours notice.
